Missing log file, missing screenshots (3 ingame screens are required for playable reports, 1 for intro/ingame), and incorrect thread title you need to use square brackets around the game ID. Yep, thanks. There's no need to include the log in the screenshots though since you're attaching it as a file anyway. Just use alt+prntscreen to take a screenshot only of the game window.
