Please add the list of LLE modules as per the submission guidelines. According to the log you seem to have used some.

NPUB30702(gta iv psn us) stops at
F {PPU[0x1000000] Thread (main_thread) [0x00365560]} struct vm::access_violation thrown: Access violation reading address 0x14

Yeah, same for me, but this must be the disc version.
